This program provides assessment, education and follow up by a team of health care professionals for clients who have gestational diabetes, prediabetes, type 1 diabetes and type 2 diabetes. The chronic disease management team; links clients to other health care professional which may include but not limited to dietitians, nursing, occupational therapist, and social worker, to ensure optimal health and well being of clients living with diabetes.
Referrals to the program can be made through physicians, other health care providers or self referral.
