Primary Care is the backbone of the health system in Alberta, 80% of all care is delivered at the primary care level. The Northwest PCN provides primary health care to more than 23,000 people living in Northwest Alberta.

  • Education on how to look after yourself with diabetes and ways to reduce risk and prevent complications from diabetes
  • Care Plans and Follow up care
  • Access to educational workshops related to diabetes self-care
